New Patient Paperwork

Call us at (623) 270-7441 and we'll send you your new patient paperwork before your first visit.

On your first visit, please bring your insurance card(s), a photo ID, and a list of your medications. If you couldn't fill out the paperwork ahead of time, arrive 10–15 minutes early.
